Tryout waiver & consent

By registering for Pelham Panthers Basketball tryouts, the parent/guardian confirms that the information provided is accurate and acknowledges that participation in basketball activities involves inherent risk.

The parent/guardian accepts responsibility for the player’s participation and agrees to follow all club, facility, and program expectations.

Additional waiver and consent details may be updated by Pelham Panthers Basketball as required.
